PARENT/GUARDIAN CONSENT FORM - EAST END BOYS CLUB

PERMISSION TO PARTICIPATE IN YOUTH LEADERSHIP ACTIVITIES

To Parents and Guardians:

 

The purpose of this form is to inform you about proposed activities/excursions involving your child, and to seek your support and permission for your child to participate.

​

This is an important document. Please review its contents carefully prior to providing permission for your child to participate in these excursions.

​

​

Activities/Excursions:

​​

  • Meetings

    • Online via various providers (Zoom, Google Hangouts, Microsoft Teams/skype, etc.)

    • In person: Templeton Secondary school or St. Francis of Assisi Basement. 

​

  • Excursions:

    • Rogers Arena (hockey games, concerts, lessons in the locker room)

    • leadership breakfasts/lunch/dinners (Vancouver Club and other host venues/restaurants)

    • cultural activities such as totem pole carvings, art galleries etc.

Travel:

​

Youth will be fully supervised at all times during excursions; however, they are expected to organize their own methods of travel to and from locations.

​

​

Educational Purpose of the Excursion:

​

These enrichment excursions are designed to be social and cultural experiences to enhance their social, emotional, and mental growth.

Supervision:

​

Supervision will be provided by one or more of our EEBC mentors and affiliated partners.

 

​

Cost of Trips:

​

Costs for these excursions will be covered by the East End Boys Club

​

Youth who participate in excursions are expected to follow and abide by the EEBC Code of Conduct

Parent/Guardian Permission:

​

While staff and mentors will take reasonable steps to prevent injuries to the youth, some degree of risk is inherent in the nature of these activities, and may occur with or without fault on the part of the youth, the EEBC, its staff or agents, or the facility where the activity is taking place. By allowing your child to participate in these activities, you are agreeing that the activities described above are suitable for your child, and that there is a risk of injury associated with the activities. Therefore: 

 

  1. THE UNDERSIGNED HEREBY RELEASES, WAVES, DISCHARGES AND COVENANTS NOT TO SUE the EEBC, its directors, officers, employees, mentors and agents (hereinafter referred to as “releasees”) from all liability to the undersigned, his personal representatives, assigns, heirs, and next of kin for any loss or damage, and any claim or demands on account of injury or death to the person participating in the program, whether caused by the negligence of the releasees or otherwise while the undersigned is in, upon, or about the premises or any facilities or equipment therein.
     

  2. THE UNDERSIGNED HEREBY AGREES TO INDEMNIFY AND SAVE AND HOLD HARMLESS the releasees and each of them from any loss, liability, damage, or cost they may incur due to an injury or accident.
     

  3. THE UNDERSIGNED HEREBY ASSUMES FULL RESPONSIBILITY FOR AND RISK OF BODILY INJURY, DEATH, OR PROPERTY DAMAGE due to negligence of releasees or otherwise while in, about, or upon the premises of the EEBC and/or while using the premises or any facilities or equipment theron on participating in any program affiliated with the EEBC.
     

  4. THE UNDERSIGNED HEREBY AUTHORIZES THE EEBC to use photographs, slides, video tapes or digital images of the person named in this release for its records, public relations or internet programs.
     

THE UNDERSIGNED further expressly agrees that the forgoing RELEASE, WAIVER AND INDEMNITY AGREEMENT is intended to be as broad and inclusive as is permitted by the law and that if any portion thereof is held invalid, it is agreed that the balance shall, notwithstanding, continue in full legal force and effect. 

 

THE UNDERSIGNED HAS READ AND VOLUNTARILY SIGNS THE RELEASE AND WAIVER OF LIABILITY AND INDEMNITY AGREEMENT, and further agrees that no oral representations, statements, or inducement apart from the foregoing written agreement have been made.
